Eligibility and Referral

Wraparound

Wraparound services are designed for families when there is a high need for intensive mental health services and multi-system integrated care management.

Eligibility: 

  1. Axis I Mental Health Diagnosis
  2. Minimum CAFAS score of 110
  3. Multi-system involvement: Mental Health and School Support, Child Welfare, or Juvenile Justice
  4. Age 5-17

Additional consideration for youth with CAFAS scores of 80-110 include: age 12 and under, three or more systems involved, history of out-of-home placement, etc.  For these circumstances please consult with your supervisor and/or the CFP Service Coordinator, Stephanie Myers, prior to making a referral. Parent Support Partner Services

Eligibility:

  • Axis I Mental Health Diagnosis
  • Minimum CAFAS of 50
  • Multi-system involvement: Mental Health and School Support, Child Welfare, or Juvenile Justice
  • Age 5-17

 

 CFP services are voluntary for families and they may choose to terminate services at any time. Eligible families may be enrolled in both Wraparound and PSP, or only one of these depending on family preference.

Referral Information:

The CFP Service referral process is simple and in line with processes that already exist in our systems. 

  1. Contact the mental health liaison in your system to review eligibility
  2. Consult with family to determine interest
  3. Fill out “Section 1” of the CFP Service Referral Form, found online at www.cfpsystemofcare.org under "Tools and Resources" tab on the "Services" page
  4. Fax to mental health liaison
